Thank you for choosing me to glam you up on such a special day! To help the day run as smooth as possible, I have included some helpful information here!

I accept cash, credit cards and Venmo. You can pay anytime between now and when you are sitting in my chair. If you would like to pay with Venmo, you can find me at https://venmo.com/dara-calzad a, please be sure to include the date of your event. Gratuity is never expected, but always appreciated!

I have two consent forms for you to fill out. The general consent form, below, can be filled out at your earliest convenience, but save the COVID-19 form for the morning of your appointment.

I acknowledge and agree that arriving with a clean, moisturized face prior to my makeup application will yield the best, most long-lasting results. I also acknowledge that certain ingredients in skincare products, including but not limited to: salicylic acid, glycolic acid, and retinols may affect the longevity of the makeup. This can be improved by discontinuing use at least 3 days prior to makeup application. Waxing, threading, chemical peels, laser services, or microdermabrasion treatments should also not be scheduled for the 3 days leading up to the event. If you receive a spray tan, we recommend having your technician skip your face. Please note, some splotches or inconsistencies in your tan can be hard to fix with makeup and we may not be able to cover given placement, coloring or time constrainsts. Any spray tan or makeup coverage requests on body are up to the discretion of the artist and will incur an additional fee.*

I will disclose all allergies and/or medications/conditions that may affect the application of makeup products. If I fail to make such a disclosure, no refunds will be provided for work unable to be performed because of these conditions. In addition, Makeup by Dara and/or its artists shall not be held liable for any unforeseen allergic reactions caused by any products used by Makeup by Dara, including, without limitation, cosmetics, lotions, oils, false eyelashes, eyelash glue, or makeup tools. I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions.*
Consent*
Makeup by Dara reserves the right to refuse services if unsanitary conditions exist and/or contagious infections are present or suspected. I understand and acknowledge that Artists will NOT provide services where any contagious infections (e.g., flu, fever, pink eye (conjunctivitis), cold sores/fever blisters, rash) are present. I also acknowledge that there will be no refund given for work performed and that the minimum fees will still be due. *
